What is Clomid?
Clomid, with the generic name clomifene or clomiphene, is in a class of fertility drugs called selective estrogen receptor modulators (SERM). This fertility medication is a non-steroid drug taken orally. When a woman has polycystic ovarian syndrome (PCOS) or another medical condition that is affecting her fertility, doctors may prescribe this drug to induce ovulation. Releasing more eggs from the ovaries can increase the chance of fertilization.
Why do doctors prescribe this medication?
Doctors usually prescribe Clomid to treat infertility. In cases where infertility is caused by PCOS or another medical condition where the ovaries do not release many eggs, and when other causes of infertility are ruled out, the drug can help increase the number of eggs the ovaries release. When a woman is getting ready to have in vitro fertilization (IVF) treatments, doctors may also prescribe these fertility pills to increase the number of eggs an ovary releases. This makes more eggs available for harvest by the IVF medical team.
Doctors may also prescribe the drug off-label to treat males who have hypogonadism, which is a condition where their testes do not function well, affecting fertility among other things. Although doctors can treat this condition with testosterone replacement therapy, the drug can help adjust hormone levels without causing side effects like infertility.
How does the drug work?
Although there are various causes of infertility, a common cause is irregular ovulation or lack of ovulation. In anovulation, an egg is not released with every menstrual cycle, and since no egg is released, there is no chance of an egg being fertilized. This medication helps increase the number and frequency of eggs released from the ovaries, increasing the chance of an egg being fertilized. It does this by blocking estrogen hormone receptors in the hypothalamus, so the estrogen does not prevent the release of gonadotropins, which are hormones that regulate the ovaries. With more gonadotropins present in the body, the ovaries will be more likely to release an egg which can then potentially be fertilized.
When to take
Your doctor will give you instructions on when to take tablets, and how long you should take it. Doctors will not usually prescribe Clomid for use longer than three to six treatment cycles because the manufacturer does not recommend long-term use of this fertility medication. Your doctor will likely instruct you to take tablets for five days beginning on the fifth day of your menstrual period. Ovulation should usually occur within five to ten days of taking this medication, and an ovulation test or a temperature check should be able to confirm this. You should have sexual intercourse while ovulating so that the released egg is potentially fertilized. If a pregnancy does not occur with the first Clomid treatment, your doctor may instruct you to take another five-day course of the medication.
Common side effects include
- Breast pain and tenderness
- Breakthrough bleeding
- Flushing with a warm, red, or tingly feeling
More serious side effects include
- Seeing flashes of light
- Increased sensitivity to light, or other vision problems
- Enlargement of the pelvic area
- Pelvic pain
- Heavy vaginal bleeding
If any of these more serious side effects occur while taking the medication, talk to your doctor immediately. While taking this medicine, especially after the first treatment, some women may develop a potentially life-threatening medical condition called ovarian hyperstimulation syndrome (OHSS).
The symptoms of OHSS include
- Little to no urination
- Rapid heart rate
- Pain while breathing
- Shortness of breath, especially while lying down
- Rapid weight gain, especially in the face and stomach area
- Stomach pain and bloating
If any of these OHSS symptoms occur, talk to your doctor immediately. These may not be all the possible side effects.
You should only take pills while under a doctor’s supervision, and always have a pelvic exam before each treatment cycle. The drug may interact with other drugs or supplements, so always tell your doctor which medications and supplements you are taking. Store Clomid at room temperature away from heat, light, and moisture.
Other medications prescribed with it
Doctors may prescribe Clomid along with Metformin when treating polycystic ovarian syndrome (PCOS). Although Metformin is originally a diabetes treatment, some patients find success when Clomid and Metformin are combined to treat infertility. This is because PCOS may sometimes be caused by excess insulin in the body, and Metformin helps regulate insulin levels. Doctors may also combine these tablets with gonadotropin injections to help stimulate the release of eggs. Other accessories, such as a fertility monitor or ovulation tests, can help determine when ovulation occurs and therefore the best time to conceive.
What is polycystic ovarian syndrome?
Polycystic ovarian syndrome, or PCOS, is a common endocrine system disorder. When a woman’s hormones are out of balance, she may experience irregular menstrual periods, an increase in male hormones, and ovarian cysts which enlarge the ovaries. Because it affects ovaries and ovulation, PCOS is one of the common causes of
